New Patients

Everything you need before your first visit, so that day is about your child and not about paperwork.

Before your first visit

  1. Call us at 1234567(Example) or send a message through the contact form, and we will find a time that works.
  2. Fill out the new patient forms. We will email them to you, or download them from Policies & Forms and bring them completed.
  3. Sign a medical release. Once you sign it, we request your child’s records from your previous pediatrician. You do not have to chase anyone down.
  4. Check your insurance. We are finalizing contracts ahead of opening. Call and we will tell you exactly where things stand with your plan.

What to bring

  • Insurance card and a photo ID
  • Immunization records, if you have a copy
  • Any old records from previous offices
  • A list of medications, including vitamins and supplements
  • Your questions. Write them down, because everyone forgets at least one.

How long it takes

Plan on about 45 to 60 minutes for a first visit. We want time to go through history properly, meet your child without rushing, and answer whatever is on your mind. Follow-up visits are shorter.

Newborns

We see newborns within 24 to 72 hours of delivery. If your baby is born on a Friday, come in on Monday. If your baby is born on a Saturday or Sunday, call us first thing Monday morning and we will get you in.

Expecting a baby? Come meet us first.

Belleville Lake Pediatrics offers complimentary prenatal meet-and-greets for expecting families. You will meet Dr. Maidlow, tour the practice, learn about our office policies and what makes our practice unique, and have plenty of time to ask questions. We will also send you home with helpful information on newborn care essentials so you can feel a little more prepared for your baby’s arrival.

Meet and greets for everyone

You do not have to be expecting. Any family deciding on a pediatrician is welcome to come in, see the office, and meet Dr. Maidlow before committing.

Switching from another practice?

It is easier than people expect. Sign one release form and we handle the rest. You do not need to tell your old office anything.

We open January 4, 2027, and we are accepting new patients now.

Newborns through age 21. Call to get on the schedule, or book a free prenatal meet-and-greet if your baby is on the way.
